The Unexpected Side Effects of NOT Taking Progesterone

It usually starts with something small. A night where you can’t seem to get to sleep, even though you’re exhausted. A wave of anxiety hits you unexpectedly in the middle of an ordinary afternoon. You start to notice a few extra pounds start showing without any explanation and feel much harder to remove, no matter what you do.

Then it builds. Your irritability gets sharper, sometimes even surprising you. The brain fog you have feels thicker. Intimacy simply becomes an afterthought, because your interest is nowhere close to what it was. And somewhere along the way, you start to wonder if this is it. This is what getting older feels like. That’s what you’ve been told, so it must be true.

At our Virginia Beach clinic, I hear some variation of this story all the time. They feel like their body is actively working against them, regardless of what they do, or what they eat. More often than not, the missing piece to the puzzle is one they really didn’t have a conversation about. Progesterone.

Most conversations about women’s hormones center around estrogen. But progesterone is quietly foundational to how you sleep, how you feel emotionally, how your body manages weight, and how clearly you think. When it starts to drop, the effects reach much further than most women expect. And key problem is that it is almost always the first hormone to decline

So you may be wondering, what is it that happens to you when progesterone goes missing? Not the clinical side effects of a medication, but the real, daily consequences of living without enough of a hormone your body was designed to depend on.

The Side Effects of Low Progesterone

When I say “side effects,” I’m talking about what your body experiences when it’s running without adequate progesterone. The symptoms that you accumulate gradually often get attributed to stress or aging, and rarely get traced back to where they actually begin.

Progesterone supports your brain’s GABA receptors (Gamma-aminobutyric acid), which is the same calming system that anti-anxiety medications target. When progesterone declines, that natural calming mechanism weakens. The result isn’t typically dramatic at first. It’s a slow erosion:  maybe a low hum of unease that wasn’t there before; then a shorter fuse with people you love; and finally a sense of emotional fragility that feels unfamiliar and unsettling.

Sleep is often the next thing to shift. Progesterone plays a direct role in sleep architecture—specifically the deeper, more restorative stages. Without it, women describe waking at 2 or 3 a.m. with a racing mind and no ability to fall back asleep. Not occasionally. Nightly.

Then there’s the weight. Progesterone influences how your body processes and stores fat, particularly through its relationship with cortisol and estrogen. When progesterone drops and estrogen becomes comparatively dominant, the body shifts into a pattern of holding weight, especially around the midsection.  This stubborn weight often feels like it refuses to budge, and doesn’t respond to any of your previous weight loss methods that used to work.

The full picture often includes several of the following at once:

  • Persistent anxiety, irritability, or mood shifts that feel disproportionate to circumstances
  • Difficulty falling asleep or staying asleep, particularly in the early morning hours
  • Unexplained weight gain, especially around the abdomen, that doesn’t respond to diet or exercise
  • Mental fog: difficulty concentrating, forgetting words mid-sentence, losing your train of thought
  • A noticeable decline in libido or a sense of emotional disconnection from intimacy
  • Heavier or more irregular menstrual cycles, or worsening PMS symptoms

What makes this particularly frustrating is how interconnected and cyclical these symptoms are. Poor sleep worsens anxiety. Anxiety increases cortisol. Elevated cortisol promotes fat storage. Excess fat storage contributes to poor sleep. And all of it traces back to the same quiet deficit that nobody tested for.

The Side Effect of Being Overlooked

Here is something that deserves more attention: progesterone is nearly always the first reproductive hormone to decline, often years before estrogen shows any measurable change. This means women can present with clear, progesterone-driven symptoms, such as anxiety, insomnia, weight resistance, cycle irregularity, and still receive a standard blood panel that gets overlooked as “normal.”

The reason is straightforward. Most routine panels don’t include progesterone. And when they do, the timing of the blood draw actually matters enormously.  Your progesterone levels fluctuate throughout the menstrual cycle and need to be measured at the right point to mean anything clinically useful.

What happens next is predictable. A woman describes feeling anxious and not sleeping, and she’s offered an SSRI or a sleep aid. She mentions weight gain, and she’s told to adjust her diet and exercise, or even prematurely recommended a weight loss medication without knowing the real underlying reason. Each symptom gets treated in isolation, and the underlying hormonal shift goes unaddressed—sometimes for years.

The 2002 Women’s Health Initiative trial generated headlines about the dangers of hormone replacement, and an entire generation of physicians pulled back from prescribing it. The consequences were significant: as of 2020, an estimated 41 million American women between 45 and 64 were candidates for hormone therapy, yet only about 2 million were receiving it. Think about that for a second.  That is 1 in 20.  Only an estimated 5% of women were being properly helped.

What followed was years of reexamination. Researchers identified serious methodological problems with the original WHI findings, particularly around the age of participants, the types of hormones used, and how results were communicated to the public. The study tested synthetic hormones in women who were, on average, well past the window where therapy is most beneficial, and the findings were generalized to all women, at all ages, using all formulations.

In February 2026, the FDA formally removed the black boxed warnings from several hormone therapy products,  including progesterone (like Prometrium), citing updated evidence that women who begin therapy within ten years of menopause onset see meaningful reductions in mortality and fracture risk. 

The broader point is this: many women were denied access to a therapy that could have meaningfully improved their quality of life, not because the science supported that denial, but because the conversation around it was shaped by fear rather than evidence.

The Side Effects of Restoring Progesterone

If you’ve read this far wondering whether progesterone itself comes with side effects, that’s a fair and important question.

Bioidentical progesterone, which is the type that is molecularly identical to what your body produces naturally, is generally well tolerated. The most commonly reported effect is mild drowsiness, which is actually why it’s prescribed at bedtime. For many women, that drowsiness becomes a benefit: it’s the first full night of sleep they’ve had in months.

Some women notice temporary bloating or breast tenderness during the initial adjustment period. These effects typically settle within a few weeks as the body recalibrates. Importantly, bioidentical progesterone behaves differently than synthetic progestins, which are the compounds used in older hormone formulations that often carried a much different side effect profile and contributed to much of the historical concern around hormone therapy.

What I hear most often from patients after progesterone is restored is not a list of side effects. It’s something closer to recognition. Women describe sleeping through the night for the first time in years. They describe a steadiness in their mood that had been absent so long they’d forgotten what it felt like. They describe weight finally responding to the effort they were already putting in.

These aren’t exaggerations. They’re the predictable result of giving the body back something it was missing.

How do I know if my progesterone is low?

The symptoms described throughout this article, such sleep disruption, anxiety, unexplained weight gain, brain fog, cycle changes are all common indicators. However, the only definitive way to confirm low progesterone is through bloodwork, ideally drawn during the luteal phase of your cycle (the second half, roughly days 19–22) when progesterone should be at its peak. A comprehensive hormone panel that includes progesterone, estrogen, testosterone, thyroid, and cortisol provides the clearest picture. If your current provider hasn’t tested progesterone specifically, it’s worth asking them to check it out, or seeking a provider who includes it as part of a thorough evaluation.

Is progesterone therapy safe?

Bioidentical progesterone has a strong safety profile, particularly when monitored by an experienced provider. In February 2026, the FDA removed its most prominent safety warnings from several hormone therapy products, including progesterone, following a comprehensive review of the scientific literature. Research supports that women who begin hormone therapy within ten years of menopause onset experience favorable outcomes, including reductions in all-cause mortality and fracture risk. As with any therapy, individual factors matter, which is why a personalized clinical evaluation is always the right starting point.
