Here are the primary mechanisms that can impact your weight:

Insulin Resistance 

Insulin is the hormone responsible for moving sugar from your bloodstream into your cells for energy. When you develop insulin resistance, your cells stop responding effectively to insulin’s signals. So your pancreas produces more and more insulin to try to get the job done.

Here’s the problem: elevated insulin is a powerful fat storage signal. When insulin is chronically high, your body is essentially locked in storage mode. You can’t efficiently burn fat for energy because insulin is telling your cells to hold onto it.

You can eat in a calorie deficit, but if your insulin levels remain elevated due to resistance, your body will fight to preserve fat stores. You might lose some water weight initially, but the fat stays stubbornly in place. Worse, you’ll feel hungry, tired, and irritable because your cells aren’t getting the energy they need even though you have plenty stored as fat.

For men, testosterone decline has similarly profound effects. Testosterone helps maintain lean muscle mass, which is metabolically active tissue that burns calories even at rest. When testosterone levels drop, muscle mass decreases and fat mass increases, particularly visceral fat around the organs.

Lower testosterone also affects motivation, energy levels, and recovery from exercise. So not only is your body composition shifting unfavorably, but you also have less drive and capacity to exercise intensely enough to counteract it. It becomes a vicious cycle.

Chronic Low-Grade Inflammation

Inflammation isn’t always obvious. You don’t necessarily feel inflamed. But chronic low-grade inflammation, often measured through markers like C-reactive protein (CRP), creates an internal environment that promotes weight gain and prevents weight loss.

Inflammation interferes with insulin signaling, worsening insulin resistance. It disrupts leptin, the hormone that tells your brain you’re full, leading to increased hunger and cravings. It impairs thyroid hormone conversion, so even if your thyroid is producing hormones, they’re not getting activated properly at the cellular level.

Inflammation can stem from poor sleep, chronic stress, gut health issues, environmental toxins, or even the inflammatory signals produced by excess visceral fat itself. Once it takes hold, it creates a self-perpetuating cycle that makes every other hormonal imbalance worse.

Why Blood Work Might Miss the Problem

You’ve probably had blood work done. Maybe your doctor ordered a “comprehensive metabolic panel” and told you everything looked fine. Maybe they checked your thyroid with a TSH test and said you’re “within normal limits.”

Here’s what that standard workup usually misses:

Fasting insulin. Most doctors only check fasting glucose, which doesn’t reveal insulin resistance until it’s already progressed significantly. Your glucose can be normal while your insulin is three times higher than it should be, locking you in fat storage mode.

Full thyroid panel. Standard testing only measures TSH, maybe T4. But optimal thyroid function requires looking at free T3 (the active form), reverse T3 (which can block thyroid action), and thyroid antibodies (which indicate autoimmune thyroid disease). Without this complete picture, subclinical dysfunction goes undetected.

Sex hormone levels. Many primary care physicians don’t routinely check testosterone in men unless symptoms are severe, and they rarely check estrogen, progesterone, or testosterone in women unless they’re explicitly seeking hormone replacement. Yet these hormones are foundational to metabolic health.

Inflammatory markers. High-sensitivity CRP, homocysteine, and other markers of inflammation are not part of routine screening, but they provide crucial insight into what’s driving metabolic dysfunction.
