The Black Box Is Gone: A Victory for Women’s Health, 20 Years Late

This week, millions of Americans are focused on Black Friday deals. But in the medical community, we are celebrating the removal of a different kind of “black” label, one that has cost women their quality of life for far too long.

The FDA has officially removed the “black box” warning from hormone therapy.

For over 20 years, this warning label has been the stop sign for women’s health. It terrified patients, paralyzed doctors, and created a generation of women who were told that suffering through menopause was “safer” than treating it.

At Optimized Hormones, we have known the truth for years. And while I am relieved the FDA has finally caught up, I also feel a deep frustration for the millions of women who were denied help because of a decades-old mistake.

The Study That Broke Women’s Trust

To understand why this news is so huge, you have to understand how we got here.

In 2002, the Women’s Health Initiative (WHI) released a study that linked hormone therapy to breast cancer and heart disease. The panic was immediate. The FDA slapped a “black box” warning (their strictest safety warning) on hormone medications. Prescriptions for hormone medications plummeted overnight. 

But the study was flawed from the start.

The WHI study focused largely on women who were already well past menopause, where the average age of the study group was 63. Even worse, the study was based on the use of Premarin, which is a synthetic hormone derived from pregnant horse urine rather than bioidentical ones.

Using synthetic drugs on older women with pre-existing conditions does not prove that hormone therapy is dangerous for the average 45-year-old woman entering perimenopause. It only proves that timing and quality of the hormones matter.

But the damage to people’s perception was unfortunately already done. For over two decades,  it was thought to be safer to just ignore and tolerate menopausal symptoms that get the quality care women deserve.

I have sat with countless women at our Virginia Beach clinic who expressed deep concern about starting hormone treatment because of what they heard from friends, family, and even other medical doctors based on what happened 20 years ago. However, this victory comes with a caveat.

While the FDA has validated the safety of hormone therapy, the standard medical system is still playing catch-up. Many providers are still trained on the old fears. They may still treat you solely based on “normal” lab ranges instead of optimal solutions.
